Abstract

This book is a comprehensive guide to lung cancer screening for clinicians, healthcare systems, community leaders, and public health officials with the hope of creating a more equitable landscape in both lung cancer screening and lung cancer-related outcomes, at local, state, and national levels. Authors take a new approach to primary and secondary lung cancer prevention that is in the early stages of adoption in the United States. The last decade ushered in recognition of screening as an effective intervention, but unfortunately, despite the wide acceptance of the importance of this new screening modality, nationally, not more than 5% of eligible subjects have undergone screening to date in the United States, although in some states uptake has reached as high as 16%. As is common with any new preventive cancer screening, racial and socioeconomic disparities emerge in utilization, stage at diagnosis, and mortality. Over time, these disparities decline, but consequential differences endure. Therefore, it is critical to establish equitable screening practices. The true measure of the effectiveness of any lung cancer screening program needs to be viewed through the lens of its impact on populations, including those most affected by the morbidity and mortality of smoking-related illness and lung cancer. As such, this book emphasizes a number of important public health topics, including community outreach to vulnerable populations, social justice issues, addressing stigma and fatalism in the general community, and the use of geocoding to assess a program’s impact at a population level. This book weaves traditional topics related to lung cancer screening, such as promoting initial and repeat screening, interpreting Lung RADs, and managing the follow-up of findings, into the population perspective in order to present a unified, comprehensive approach to the subject. Further, it serves as a guide that health systems, health care professionals, community leaders, and other stakeholders can use to achieve the promise of lung cancer screening.

Abstract

Great strides have been made in the area of hydrogel science since the first hydrogels were described in the 1960s. Hydrogels usually consist of three-dimensional matrices of hydrophilic polymers, held together by chemical or physical crosslinks, or supramolecular assemblies of small amphiphilic molecules. The gelation process occurs in response to a physical or chemical stimulus, such as temperature, pH, electric or magnetic field, enzymatic modification, light, and others. Consisting of mainly water molecules, they represent a unique class of materials, with many applications such as cell therapeutics, cartilage/bone regeneration, sustained drug release and drug delivery systems, tissue engineering, and 3D bioprinting. Despite these great strides, there is still much more to discover in this area. This Special Issue is focused on the use of hydrogels in tissue and bone regeneration. Hydrogels are particularly suited for this purpose as their physical characteristics resemble that of the extracellular matrix; as such, they have found applications as an extracellular medium for cancer cells, stem cells, and neuronal cells. This Special Issue also includes research papers on the other biomedical applications of hydrogels.

Abstract

Why did some doctors in Classical Greece feel compelled to study the universe as a whole? How could cosmological principles be employed in clinical practice? This book explores the works of the cosmological doctors, such as On Breaths, On Flesh, and On Regimen, and argues that they form part of a much broader reorganization of medical knowledge in the fifth and fourth centuries BCE. These healers used cosmological principles as a supplement to, rather than a replacement of, more traditional approaches to health and disease, creating theories about the cosmos whose obscurities can best be understood as the products of medical thinking. The intensive care unit (ICU) is a specialised hospital ward where the ‘sickest-of-the-sick’ patients, often with life-threatening illness, receive around-the-clock monitoring and life support. There is a wide spectrum of conditions managed, and these present unique challenges for those who work in this field. Abstract

This book reviews, in one place, the breadth of articles that form the basis for the current practice of medicine. When physicians examine medical literature, there are articles that are quoted over and over again, as well as articles that have established the standards that are so embedded that we no longer think of where those standards came from. When clinicians treat patients with diabetes, why do we aim for an A1c less than 7%?
